Hello, University of Delaware. I'm Tara Leonard, a clinical instructor, Health Coach, and Center Director of UD Health's Clinical Health Coaching Research and Training Center. I'm excited to offer you a brief overview of our services, where we're located, and how to connect with us. First, our health coaching home is located in Star Tower on South Campus. The tower is right across from UD Field House. We're located on the second floor of the tower. We work collaboratively with several of UD health clinics, including Nurse Managed Primary Care, the Nutrition Clinic, and Exercise Counseling. This offers us an opportunity to provide continuity of care across multiple disciplines to you. Clinical health coaching is available to all UD staff, employees, as well as community members. At this time, clinical health coaching is offered by board certified health coaches or graduate student health coaches learning under us. As a center, we offer collaborative programming in a group format. Current programming includes the CDC's National Diabetes Prevention Program, ANEW, which is Aligning Nutrition, Exercise, and Wellbeing, and then in addition, Family ANEW, to help people living under the same roof with shared goals to support a healthier lifestyle. These programs are offered at various times during the year. Additionally, we offer individual health coaching sessions. Prior to the pandemic, we offered individual and group health coaching sessions in person or virtually. Currently, all of our sessions are conducted virtually or on the phone. We hope to be face to face in the very near future. We miss seeing all of you. You may be asking, "What's health coaching?" So, I'll take a moment and offer a brief description. Health coaching is client-friendly term for health behavior change system. Or another way to say it, health coaches are facilitators of positive change. As coaches, we explore our clients' readiness to change and we focus on what they can actively do to improve their health. The ultimate goal is to support and empower our client to take responsibility for their own health management in a positive way. The focus may be in the context of self-management, or in some cases, disease prevention or chronic condition management. We meet clients where they are, shoulder-to-shoulder to help them discover how to make positive and exciting changes to their health. Emphasis is on self-directed, targeted goal setting with small changes at a time. We encourage progress, not perfection. Also, we offer resources or professional referrals to ensure clients are informed of all available support opportunities. Often times, a client focuses better adherence or the recommendations from a personal health care provider such as a prescribed medication regimen, diet, or exercise program. For example, an RDN from the Nutrition Clinic may be working with a client on a dietary protocol and the collaboration with the health coach can add an extra layer of support or accountability with individual sessions or within a group program. We can also offer assistance in areas of sleep, hydration, stress, or time management. And as we all know a little bit of work-life balance. For individual coaching sessions, participation involves six sessions, typically every two weeks over a 12-week period of time. However, there's some flexibility depending on your needs. The first remote session typically lasts about an hour, with subsequent sessions lasting 30 minutes.
